First step: identifying every possible risk. We document 30-50 possible scenarios per event.
Risk assessment matrix
Every risk assessed on two dimensions: likelihood (rare, low, medium, high, almost certain) and impact (minor, light, medium, major, catastrophic). We multiply dimensions for a risk score (1-25). Risks with score 15+ need deep mitigation, 8-14 standard mitigation, 1-7 monitoring only. Matrix guides focus.
